Waiver of Liability: Eagledove Martial Arts & Mountain Island Fitness

Recognizing the practice of Tang Soo Do and other martial arts, as well as the use of Eagledove Martial Arts facilities, services, and programs, involves an inherent risk of personal injury to Student and Student’s guests and invitees, I understand that I am voluntarily participating in an activity which has bodily contact and I do so entirely at my own risk.  I hereby agree that reasonable care is taken to prevent serious injuries and to minimize accidents and understand the requirements that students must obey the rules and regulations of said instruction.

Waiver of liability; assumption of risk: I voluntarily agree to assume all risks of personal injury, illness, or death to myself, my spouse, children, unborn children, other family members, guests, or invitees. I agree to waive any and all claims or actions that I may have against Mountain Island Fitness, Eagledove Martial Arts, MBF Ventures, and any and all instructors, students, and participants, and to hold such entities and individuals free and harmless of any liability for damages of injuries arising from my participation in Tang Soo Do classes and other activities including, but not limited to: free sparring, demonstrations, tournament competition, falling, being struck by a fellow practitioner, use of equipment and all amenities, and supervised or unsupervised activities and programs on the premises, including in restrooms and dressing rooms. This assumption of risk includes any injuries and medical disorders such as heart attacks, strokes, heat stress, sprains, broken bones and torn or damaged muscles, ligaments, or tendons, and accidental injuries; and any injury, illness, or death resulting from the presence of or transmission of any bacteria, viruses, or infectious diseases.

I further acknowledge that Mountain Island Fitness, Eagledove Martial Arts, MBF Ventures, nor its assigns or affiliates, instructors, employees, agents shall be held liable for any defective equipment.

I affirm I am fully responsible for any and all medical expenses that I may incur and am responsible for carrying my own insurance.  

Liability for personal property: I hereby agree that neither Eagledove Martial Arts nor Mountain Island Fitness shall be liable to me or any of my guests or invitees for any personal property that is damaged, lost or stolen while I am participating in activities with Eagledove Martial Arts at Mountain Island Fitness, including, but not limited to, a vehicle or its contents or any property left in a storage container.

Waiver of Photographic Rights: I hereby agree that any pictures taken of or by me in connection with Eagledove Martial Arts classes, demonstrations, seminars, or tournaments may be used by Eagledove Martial Arts for publicity or promotion without compensation at this or any other time.
